Consortium Goals

To develop a multi-center and multidisciplinary consortium committed to investigating person-centered outcomes in stroke survivors

To establish a network of  secondary and tertiary stroke prevention programs with an evidence driven and health equity-focused approach

To establish a multi-center registry to support research and learning healthcare initiatives within the consortium’s stroke prevention programs

To develop innovative models of care delivery, including those that incorporate technological advances and tele-health

To serve as a resource to stroke survivors, caregivers, and stakeholders committed to improving stroke outcomes

To establish guidelines for the delivery of care across the post-stroke care continuum
